Directions to our offices

1) Hollis: F Train to Jamaica 179 Street (last stop), then take Bus Q36 to 196 Street; office beside MOBIL Gas

2) Flushing: E or F Train to Kew Gardens/Union Turnpike, then take Bus Q46 to 153rd Street; OR J or Z train to Sutphin Blvd, walk two blocks to Parsons Blvd/Jamaica Ave. and take Bus Q34/25 to Union Turnpike, walk 2 blocks to office on 153rd Street; or F train to Parsons Blvd, then take Q34/25 OR #7 Train to Flushing, take Bus Q25/34.
 

3) Hill: J or Z Train to 104 Street, walk one block to office OR Bus Q 56 to 105 Street.

4) Brooklyn: J Train to Pennsylvania Avenue, then take Bus B20 to New Lots Avenue; or #3 Train to Pennsylvania Avenue and then take Bus B20. Office located between New Lots and Hegeman Avenue

• No Insurance? No problem. YOU CAN NOW SIGN UP FOR UNLIMITED OFFICE VISITS FOR ONE YEAR FOR AS LITTLE AS $399 PER YEAR!

• MAIN OFFICE: 79-35 153RD STREET, FLUSHING, NY 11367. (718) 591-1600.  Our patients can visit any of our 5 locations. Discounted fees for those without insurance. All Offices open 10 am to 6 pm M-F; and 9am - 3pm on Saturdays. The Pennsylvania Avenue Brooklyn Office is open at 8am on Mondays. The Flushing office is open 10am - 3pm Sundays. These hours are subject to change due to Provider's vacation etc, therefore please try to call the office number  to confirm opening hours  before  you  leave  your  home

• WALK-INS ARE WELCOME ANYTIME, but please make appointments if you can.

• Unlimited Doctor’s Visits For One Whole Year :$399 for Individual; $599 for Husband & Wife; $799 for Entire Family

• This is not medical insurance, but pre-payment for Doctor’s visits at considerable savings to you. Only Doctor’s visits are covered in the Pre-Paid Plan. One single payment covers as many visits as you need in one calendar year to any of our Family Doctors in our 5 offices in Queens and Brooklyn. Any other services needed, including: PPD, Labs, Vaccines, EKG, PFT, Audio etc. will be at extra charge. You must pay for the entire year at time of registration by cash, check or credit card. 30 days advance notice required for cancellation. Unused portion of payment will be refunded pro-rata.
